1.0 STATEMENT OF PURPOSE
L.V. Doubleday Counseling, PLLC respects and uphold an individual’s right to privacy and to protection of his or her personal information. The purpose of this statement is to let you know what personal information L.V. Doubleday Counseling, PLLC collects and holds, why collect this data, how long it is kept and your rights over your personal data.
L.V. Doubleday Counseling, PLLC is accountable for the personal information under its control and is committed to protecting your information.
2.2 Client Information
We collect personal information from you when you enquire about our counselling services in order to set up an initial appointment. This information includes contact details, your availability and other relevant personal information. Once a client finishes counselling, all data regarding their counselling is stored securely for 7 years and then destroyed.
L.V. Doubleday Counseling, PLLC will obtain the appropriate consent from individuals for the collection, use, or disclosure of their personal information, except where the law provides an exemption. We will not share your personal details with any other person or organization without your knowledge and permission, unless there is a legal requirement, if there is a child or adult safeguarding issue, or a perceived risk of harm. A breach of confidentiality is when a person shares information with another in circumstances where it is reasonable to expect that the information will be kept confidential.
2.4 Limiting Collection
The personal information collected by L.V. Doubleday Counseling, PLLC will be limited to that which is necessary for the purposes identified.
2.5 Limiting Use, Disclosure, and Retention
Personal information will not be used or disclosed for purposes other than those for which it was collected, except with the consent of the individual or as required by law. L.V. Doubleday Counseling, PLLC does not sell, trade or rent information to third parties.
Personal information collected by L.V. Doubleday Counseling, PLLC will be as accurate, complete and up-to-date as is necessary for the purpose for which it is to be used.
L.V. Doubleday Counseling, PLLC protects personal information with appropriate security safeguards. Safeguards include physical, administrative, and electronic security measures. We will take all reasonable precautions to prevent the loss, misuse or alteration of information you provide us.
Communications in connection with services provided by L.V. Doubleday Counseling, PLLC may be sent by e-mail or text message. For ease of use and compatibility, communications will not be sent in an encrypted form unless you require it and give us permission to communicate with you in that way. E-mail, unless encrypted, is not a fully secure means of communication. Text messages are also not a fully secure method of communication and may be limited in nature for security purposes. Whilst we are strongly committed to keeping our systems and communications protected against viruses and other harmful effects, we cannot bear responsibility for all communications being virus-free.
L.V. Doubleday Counseling, PLLC will provide clients specific information about its policies and procedures relating to the management of personal information which is under their control.
2.9 Individual Access
Individuals have the right to access their personal information under the control of L.V. Doubleday Counseling, PLLC. In certain exceptional situations, L.V. Doubleday Counseling, PLLC may not be able to provide access to certain personal information that it holds. If access cannot be provided, L.V. Doubleday Counseling, PLLC will submit written notification giving reasons for the refusal.
